Omeprazole and Clopidogrel

Clopidogrel (Plavix) prevents blood clots. Omeprazole (Prilosec) is a proton pump inhibitor used for heartburn. Omeprazole can reduce how well clopidogrel works.

What you need to know

Both omeprazole and clopidogrel are processed by the same liver enzyme (CYP2C19). Omeprazole competes for this enzyme, reducing the activation of clopidogrel and potentially making it less effective at preventing clots. The FDA has warned about this interaction. Your doctor may recommend a different PPI such as pantoprazole.

What to do

Do not stop clopidogrel or your PPI without talking to your doctor first.

Ask your doctor whether switching to pantoprazole would be safer for you.

Tell your cardiologist about all stomach-acid medicines you take.

When to call your doctor

Chest pain or signs of a heart attack or stroke — call 911 immediately.

Any new stomach symptoms while on this combination.
